It shouldn't. Shortchange shows the reference rate — the mid-market number that banks and exchange houses price against. Nobody trades at it. The gap between it and what you're offered is the spread, and showing you that gap in money is the entire point of the app.
The loss is measured against the reference rate, and the bands are fixed:

These are judgements, not law. An airport kiosk will beat 6% almost never; a city-centre bureau usually should.
That is the app being honest rather than broken. It only refreshes when it can reach the network, and it will never silently show you a stale number as if it were current. Tap Update once you have signal. If it still won't refresh, the rate service may be rate-limiting; it clears within about twenty minutes.
Yes — that is what it is for. A rate snapshot is compiled into the app, so a brand new install works before it has ever been online. Just watch the age shown at the top of the screen.
The app covers 64 currencies. If yours is missing, email it and it can go into the next release.
